Cover Letter clarification

Please clarify on the following...

1.
In FROM, should i give the address along with Name / phone numbers / email id or just Name / phone numbers and email id are good enough...

2.
To whom should the TO be addressed to in the Cover letter.

Note : i currently have The Assessing Officer (without address)

3.
This is the header that i currently have in the cover letter...Application for Tier 1 (General) of the Points Based System


Is this header ok to go ahead and apply ???

1. In the "From", I usually mention name and address only. Other contact details such as mail id, phnoe no etc, I provide at the end.

2. I have addressed the "To" secrtion to:

The Entry Clearance Officer,
British Deputy High Commission,
20 Anderson Road,
Chennai 600 006

Please check with others, I may be completely wrong. Also, the place depends on where you are applying.

3. Header looks fine to me.
