Tier 1 Dependant visa form Query within UK

Hi all,

I am on Tier 1 (General) visa. My daughter was born ealier this month. I want to apply for her dependant visa. Following are my queries on the form. I would really appreciate if someone can help me:

1. C1: Under which nationality did the dependant enter UK?

Since she is born in the UK so what should i say? I am pakistani passport holder so should i mention Pakistani in response or born in uk

2. D1: Did the dependant obtain entry clearance or visa before coming to the UK?

3. D5: Please enter reference number or visa which can be found in dependant passport?

4. D12: What is dependant current immigration status?

5. Do I need to send my own passport as well in her application or a photocopy will be sufficient?

I will really appreciate guidance.

just put N/A or Born in the UK

err i forgot that, i think you can send a copy but search the forum these questions have been answered many times before.
I will really appreciate guidance.

I know these answers look weird but then again they haven't done the form smartly enough to be used for a child born in the uk
