We just submitted the online form for my wife's application and her name is messed up. It asked for other names by which she had been known so we put her full maiden name. It never looked wrong as displayed on the preview page until we got the booking appointment for biometrics page and then it became very clear that we were only supposed to put her maiden name. At this point we could not go back to change it. Here is how it looked when filling in the form (with First, Middle, etc substituted for her real name):
Given Name: First Middle
Family Name: Last
Other Names: First Middle Maiden
Here is what it looks like on the booking appointment and header on the application itself:

I am really worried that we will not be able to successfully complete this application with the name this way, but unfortunately we have paid already. Is there anything we can do? I can correct it by hand but I'm worried that Homeland Security will not accept her passport as identification for taking the biometrics because of this error. She does have her previous passport which reflects her name change, so maybe that is enough?
