Tier-1 application form filling help

Hi all,

My Tier-1 Visa is about to expire and I have to apply for an extension. I was going through the form and I could not understand some of the questions in the form. If any one of you could help me with them, it will be gr8.

1. First query is regarding question D1. I entered UK in 2007 on a Work Permit and later switched to Tier-1 Visa in 2008, so if you see I entered UK for the first time after obtaining a Visa outside UK but I am currently on Tier-1 Visa, so what option should I select, Yes or No.

2. For question D4, what number on the Visa is the reference number, Visa seems to have multiple numbers, one is printed in red colour right next to text ‘UK Entry Clearance’ and there is one more number against field Type, which starts with text, ‘Visa Work Permit’, followed by a number starting with ‘F’ or is it the long number at the bottom of the Visa with a lot of ‘>>>>’. Also reference number from which Visa should I enter, the reference number that is printed on my expired Work Permit Visa or current Tier-1 Visa?

3. Question D5, it’s difficult to say what the question means, I just have one passport and both my old work permit and the current Tier-1 Visa are on the same, so I assume that both my permits are available for the case worker to see and verify. I believe I should answer No here, if you guys think otherwise, please correct me.

4. What should be entered for questions B13 & B14?

5. This is regarding question O4, I worked for two employers, first one from Oct-2008 to Aug-2010, I only joined my new employer on the 20th Sept, so there is a gap of 20 days in between, so while I enter the period for my previous earnings, shall I just enter it as one period or break it into two taking the 20 days out?
