Hi

My tier 1 visa was issued in Oct 2008 and expires in Oct 2011.
I had a look at the UK BA website and could only locate the form on this page: http://www.ukba.homeoffice.gov.uk/worki ... nsionform/

However, on the actual pdf form, it says that 'This form is to be used for applications made on or after 14 December 2010'. My initial Tier 1 visa application was made on Oct 2008. Can someone please confirm if it is the correct form?

also, should i be referring to the Policy guidance notes from 6 April 2011?

The date of application is applicable to the application you will be making, not to initial/past applications. You should always use the latest form available at the time you are applying.
