ILR - Format of employment letter - after 6th Apr 2011

Hi,
Can anyone please post the format of the employment letter to be submitted along with the Set(O) ILR form?

Also if the spouse is on dependent visa, will they be asking about the work status of the spouse? Can we disclose that they are working?
