tier 1 extension dependant

hi,
i have got appointment on thu morning and this is urgent .

could u help me with following answers in tier 1 (gen)dependant form
what is

1: home office refertence number. section 2 : b11
2:points based system migrant reference number. section 2 : b14
3: reference number on the entry clearance or visa found in dependant's passport or travel document.
4:home office reference number . section 3:G5
5: pbs migrant reference number . section 3 : g6
