I am wondering, when the employer have a job ready for her, why can not they sponsor work permit for her. Why she need to have HSMP visa.
After few years working here, she may qualify for HSMP automatically.

I think she has simply made the wrong application, or rather made the HSMP application at a time when she does not have enough points.

The profession of NHS Salaried Dentist is on the occupational shortage list. That is an NHS organisation .... probably a PCT ... could get a WP for her without there being any requirement to advertise the vacancy.

After a year in such a job she might have a better chance of accumulating enough points for HSMP, or whatever is around at the time.
