Coming to the Order of presenting ur application its all up to u how u can make it more clear and understandable to Case worker(CW). Search for the post once Marcnath has posted how to present ur application n try it , bocz I liked the way he suggested to do. It can be followed to any application u make .

If u like u can simply follow :
1- Cover letter on top of all
2- Fee paid Receipts ( Easy to identify and also the payment pages come early in application form, so it gives the answer straight away)
2- Application ( use paper clip for the application, so that it will be easy for the CW to remove and put it back , how ever he/she needed )
3- Only required documents stated in Guidance comes here ( LIFE IN UK, Degree or English Test Certificate
4- Extra supporting docs (only if Needed) , dont complicate ur case , only provide them if they ask for any
