sorry to hear about your refusal, but it's good that you got to talk to your caseworker and know exactly what documents you need to provide this time.
in answer to your questions,
i think if you go with paid appeal, you don't need to submit original documents, just the additional ones. just provide the ACCA letter and the detailed job description. include with it a letter explaining why the 35 points should be awarded. send a copy of their letter to you (to be safe) so they know there's no need to re-evaluate the other sections.
fresh application - send everything again. you get the additional year of work experience and if you are still under 28 now, your application is stronger.
time scales - from what i've read on this board, both options for you would take around the same time. people who have sent second (fresh) applications seem to be put on a different queue as first time applicants, and the 2nd time ones are taking longer. appeals are also taking longer.
